The VSRF is a straightforward, easy to use, electronic template for GP referrals. It was developed to support the coordination of services between GPs and community-based services funded by the Department of Human Services. The VSRF is the standard form for GP referrals to primary care services funded by the Department of Human Services (DHS), including Community Health, Home & Community Care, Aged Care Assessment and so on. The VSRF can also be used to refer in to hospital outpatient services.
