The ‘Life! Taking Action on Diabetes’ prevention program is based on the Finnish Prevention Program and the Greater Green Triangle Diabetes Prevention Program targeting people aged 50 and above at high risk of diabetes. www.diabeteslife.org.au

The Course

 

The Life course is a lifestyle behaviour change course of six group sessions, run over a period of eight months. The course aims to help your patients work towards a weight loss goal of five per cent of total body weight through healthy eating and moderate daily exercise.

 

 

 

You can prevent type 2 diabetes by enrolling in a type 2 diabetes prevention course.  This new course is free for most people and available in your local area.  With group support, our health experts offer you a tailored lifestyle program that has proven and sustainable results. If you are finding it difficult to attend a Life! Course, the Telephone Health Coaching Service is another way you can take action to prevent or delay the onset of type 2 diabetes.

Referral Template

 

A template for Medical Director, Best Practice and GP Complete is available in a combined format for the two age groups and this single form is used by general practice to reduce confusion over which referral form to use and when. MBS Item Numbers

 

 

General Practitioners can utilise one of the following MBS health assessment item numbers:   

Item 701: Brief Health Assessment of less than 30 minutes

Item 703: Standard Health Assessment lasting 30 to less than 45 minutes

Item 705: Long Health Assessment lasting 45 to less than 59 minutes

Item 707: Prolonged Health Assessment 60 minutes or more

Annotating: either a 45-year old health assessment or a type 2 diabetes risk evaluation or 

Item 715: Aboriginal and Torres Strait Islander people’s health assessment
